In consideration of my participation as a volunteer for the Military Working Dog Heritage Museum and Handler Center, on behalf of myself and all heirs, administrators, executors and assigns, I agree to the following:
Assumption of Risk and Waiver of Liability. I hereby release and agree to indemnify and hold harmless the Military Working Dog Heritage Museum and Handler Center, its volunteers, employees, and agents, from any and all liability for any damage or injury which may occur or result, regardless of the cause, from my participation in my volunteer work. This release of liability and agreement given by me to the Military Working Dog Heritage Museum and Handler Center, its volunteers, employees, and agents, shall apply to any claim, demand, suit or right of action that might accrue to myself, my heirs and my personal representatives, including claims of negligence. Further, I agree to assume all risks associated with my participation in the volunteer activity, and I am fully aware that personal danger may be involved in this activity. I further understand that volunteer activities may be managed solely by other volunteers, and I agree to participate in the activities with full knowledge of these conditions. I agree not to seek contribution or indemnification from the Military Working Dog Heritage Museum and Handler Center, its employees or agents, if I am sued by any party in connection with my participation in volunteer work.
Media Consent. I consent to the unrestricted use of my image and/or my name, in connection with my volunteer efforts that has been approved for distribution by the Board of Directors including, but not limited to, any photographs, audio or video recordings, interviews, videotapes, or motion pictures whether for television, radio or print media. I may opt out by notification of the event coordinator and a member of the Board of Directors 48 hours in advance in writing of any event.
Travel consent. I understand that I must provide my own transportation to attend Military Working Dog Heritage Museum and Handler Center events and activities and that I assume all responsibility for costs, risks, and dangers inherent within that travel on behalf of myself and any minor children that may accompany me.
Participants. I understand that I have voluntarily agreed to assist in a volunteer event, that the date for events may be changed at the sole discretion of the Board of Directors, that I will not be paid for my services and that either the Military Working Dog Heritage Museum and Handler Center or I may terminate my participation at any time for any reason.
This document shall be binding on the heirs, executors, administrators or assignees of the undersigned or accompanying minor.
A signed counterpart of this waiver which is electronically transmitted shall constitute an original.
The MWDHM Board requires all MWDHM Volunteers to resubmit a signed copy of the Liability Waiver annually.
